STEP #1
Refer a friend to make an appointment with Dr. Cohen.
Potential patient must be new to Dr. Cohen and not been seen at this office in over five (5) years.
Potential patient can be a friend or family member.
STEP #2
When your referral checks in (either by phone or through the web form), please make sure they mention or write YOUR NAME so you can receive the credit.
If we are not told by the time the appointment is made, we can’t count it towards your referral totals.
